The Member Data Form should be used to report:
* Name/Address Changes
* Date of Birth
* Continuous Years Changes
* Deceased Members
* Post Transfers
1. Open the PDF Data Form - Click here þ
2. Input your information
3. Move from field to field by using the "TAB KEY"
4. Print two copies of page one
5. Mail one of the forms to the Department Office
